Here we discuss about patient's problems with a series of inputs with an aim to solve them ★ CASE SCENARIO  A 60 year old male patient came with   CHIEF COMPLAINTS : • Increased abdominal pain since 10 days • SOB since 10 days • Unable to speak since 2 days ★ HOPI .Apparently pt was asymptomatic 6 mnths back • Then he developed puffiness of face, pedal edema, abdominal pain, back ache, fever, burning micturition for which he visited a local hospital.  • He got diagnosed with renal calculi and also shrunken kidneys for which he took medications and got relief.  • Since 10 days, he again developed increased abdominal pain and SOB • Also couldn't speak from 2 days k/c/o renal atrophy/shrunken kidneys since 6mnths • NSAID ab

Here we discuss about patient's problems with a series of inputs with an aim to solve them ★ CASE SCENARIO A 50 YR OLD FEMALE WITH ANURIA ★ CHIEF COMPLAINTS  Anuria since 3 days Fever with chills since 3 days SOB grade 3 since 3 days Throat pain with difficulty in swallowing since 1 week ★HISTORY OF PRESENT ILLNESS -Apparently pt was asymptomatic 1 week ago Then she developed few throat ulcers which made her difficult to swallow even water.  -She got treated for it in a local hospital but it did not get cured.  Due to those ulcers, she couldn't even drink nor eat.  -Since 3 days, she developed fever with chills, anuria, SOB Yesterday morning, they visited a govt hospital in suryapet, where they stayed till evening just to

Here we discuss about patient's problems with a series of inputs with an aim to solve them ★ CASE SCENARIO  A 50 year old female  patient who is House wife came to the OPD with chief complaints of : - Nausea and vomitings since 2 days - weight loss since since  1 year  - Heavy menstrual bleeding since 1 nd half year ★ HISTORY OF PRESENT ILLNESS : - Patient was apparently asymptomatic 2 days ago and developed vomitings which are billious in nature , food as content if taken food which is preceded by severe nausea since 2 days , 20-30 episodes till the time of admission No HISTORY of fever, loose stools, pain abdomen, - patient did not take insulin on the night before admission and on morning of admission day , she is on OHA '
