           I choose my friend which is closest to my roll number as reference for reviewing a blog .

          In the first question she selected Ten different cases and explained them in very well manner . The entry of her review on the cases were well  suggested . She mentioned the links of cases in her review making it easier to go through them. She covered major systemic organs. 

          In remaining questions her work is really well appreciated . The blog done by my friend is admirable . She entered the data in a chronological order and each and every points were clearly mentioned . The information of all the investigations was entered. The treatment plan is well explained in the blog . The terms and the use were described in a understandable way.

          The work done by friend is really commendable . Her efforts are very amazing in making a blog.

*This is the case link of  AKI Of 61 year old male patient 

* The patient came to the casuality with chief complaints of lower abdominal pain , lower back ache after lifting weights , burning micturition , SOB , decreased urine output .

* He was diagnosed as AKI 2° to UTI associated with Denovo-DM-2 

* Due to AKI there is build up of waste products in the body which disturbances the normal balance

— ACUTE ON CKD

* The probable diagnosis  was Acute renal failure  ( intrinsic ) Grade 1 L4-L5  spondylodiscitis , multifocal infectious spondylodiscitis, Hyperuricemia 2° to Renal failure , uremia induced tremors ( resolved ) , Delerium 2° to septic / uremic encephalopathy ( resolving ) 

* The timespan of patient was entered according to the symptoms.

* The systemic examination of the patient was well described 

* Day to day treatment plan is mentioned

*  In the first link mentioned above the patient was unconscious when came to the hospital and all the details are said by the patient daughter , the blog consists of all the details in a chronological way , the investigations done are well described , the systemic examination of the patient was entered in an authorised form , the day by day treatment plan and the procedure of the done treatment is well mentioned.

QUESTION NO : 4 

             The above mentioned all the cases are related to renal failures .

- The renal failure denotes the failure in function of the renal system.

- Renal system ( EXCRETION SYSTEM ) plays a vital role in our body by excreting all the toxic substances from the body 

- Failure of the renal system function results in accumulation of the various harmful substances inside the body leading to different type of complications which in turn makes the living of an individual difficult.

- In most cases renal failure is caused by other health problems ( The most common cause is  DIABETES ) 

- Renal ( KIDNEY ) failure is the last stage of CHRONIC KIDNEY DISEASE        ( CKD)

- The sudden stoppage of the function of the kidney is termed as ACUTE RENAL FAILURE ( AKI ) 

- The symptoms of the ACUTE KIDNEY FAILURE includes : 

  * Diarrhea

  * Vomiting

  * Abdominal pain 

   * Back pain 

- The symptoms suggesting Renal failure include :

  * Increased or decreased urine output

  * Loss of appetite

  * Trouble in sleeping 

  * Swelling of the feet of ankles

  * Breath problems etc.

{ The above mentioned symptoms are all present in one or other case links mentioned in the QUESTION NO : 3 }

- The treatment varies for the renal diseases and  is given symptomatically 

- In the above case links the treatment given to the individual patient varies and given according to their chief complaints , there history ( present , past , Drug.. ) , resolved symptoms , reports of investigations..

                 It's been one more month going through this online clinical sessions , in this pandemic  time the faculty is trying there best in explaining all the cases arriving the hospital . They help a lot in exploring our knowledge regarding the cases . The proffesors make us to think all possible aspects of the cases. Though there are some hurdles coming on our way ,  we are trying our best to come across them. We are learning new points on daily basis through these sessions.

               We are learning how to take history from the patient and making a e-log under the guidance of our interns.Not everyone easily can go through the cause for the patients symptoms , but trying our best in knowing them .
